Varicella (Chicken Pox) Vaccine Now Required For All Children Born On Or After January 1, 1998, Entering School

Varicella (chicken pox) vaccine is now required for all children born on or after January 1, 1998, who are entering school in September 2003. The chart below lists the required immunizations for school entry.

Varicella 1 dose if born on or after January 1, 2000. (Not recommended until age 1 year.) 1 dose if born on or after 1/1/98. Recommended if no history of illness.
Diphtheria Toxoid containing vaccine (DTP, DTaP) 3 doses 3 doses Td recommended every 10 yrs.
Polio (IPV or OPV) 4 doses IPV or 3 doses OPV as age appropriate 4 doses IPV or 3 doses OPV Not applicable
Mumps 1 dose 1 dose 1 dose*
Rubella 1 dose 1 dose 1 dose*
Measles 1 dose 1 dose if born before 1985; 2 doses if born on or after 1/1/85 2 doses*
Hepatitis B 3 doses if born on or after 1/1/1995 as age appropriate 3 doses if born on or after 1/1/93 ; for all children entering seventh grade as of 9/1/00 ** Recommended for students in all health fields
Haemophilus Influenza Type B (Hib) 3 doses if the child is less than 15 months of age; or 1 dose administered on or after 15 months of age Not applicable Not applicable
NOTE: (*) Only for students born on or after 1/1/57
NOTE: (**) Alternatively, 7th graders may have 2 doses of adult hepatitis B vaccine
