Speakers include representatives from Long Island Transition Site, Parent to Parent, Suffolk Medical Home as well as the two county health departments. There also will be representatives from Care At Home, Early Intervention, Child Health Plus, Medicaid, SSI (Supplemental Security Income), OMRDD, (Office of Mental Retardation and Developmental Disabilities), VESID (Vocational and Educational Services for Individuals with Disabilities).
