Written doctor’s orders are required to receive nursing services and physical, speech or occupational therapy on a part-time intermittent basis under Medicare. Medicare only pays if the patient is homebound and is acutely ill. Medicare does not pay for care in chronic illness. If approved by Medicare, limited part-time services of a home health aide may also be provided if skilled nursing and/or therapy services are also ordered by the doctor.

A professional registered nurse makes an initial assessment in the home of the sick person and, together with the patient and family, designs a plan of care to carry out a physician’s medical treatment plan. Teaching the patient self-care and instructing family members as needed are essential services offered. Periodically, the patient’s progress and family’s ability to participate in the care are reviewed and evaluated by the home care team. Payment may come from fees paid by patient or family (determined by ability to pay), Medicare and Medicaid reimbursement for eligible patients, or insurance plan payments.
