Equipment Violation

If your violation code is a #375, a #376, or a #381 equipment violation, and you choose to make timely repairs to obtain a dismissal, you have two options. Within 24 hours of the violation, you can either submit an authorized repair shop’s proof-of-repair or you can repair the vehicle, privately or by yourself. Following the repair you must complete and mail a proof-of-repair form (PDCN –68), available from any Nassau County Police Officer or Precinct (or by using the link below), with the signed ticket (Part B) along with the authorized repair shop’s proof-of-repair or the proof of purchase for the part, to Nassau County Traffic and Parking Violations Agency (N.C.T.P.V.A.). A $30 Driver Responsibility fee is required for this dismissal request. Please submit a $30 check or money order payable to N.C.T.P.V.A. along with your documentation. If you do not repair the vehicle within 24 hours, by default you are choosing to plead guilty. Sign (Part A), attach your payment of $130 and mail all including your ticket to 16 Cooper Street West, Hempstead NY 11550-4063.

All documents must be returned on or prior to the return date on the ticket.
