AWARDS TO LOCAL PHYSICIANS – FREEPORT

Ocean Physician’s office was awarded a Certificate of Recognition from the Nassau County Department of Health for participation in Nassau County Department of Health’s Adult Immunization Program Initiative to improve adult health by promoting and encouraging the use of adult immunizations.

Dr. Peter Chak’s office for the practice of Adult Medicine is located at 33 North Ocean Avenue, Freeport, N.Y. 11520.
