End of Academic Year Celebrations During COVID-19

END OF ACADEMIC YEAR CELEBRATIONS
DURING THE COVID-19 STATE OF EMERGENCY

The New York State Department of Health Interim Guidance for End of Academic Year Celebrations During the COVID-19 State of Emergency (dated April 12, 2021) requires Responsible Parties to notify local Health Departments of any event that will exceed the State’s gathering limits.

To comply with these requirements when exceeding the social gathering limits, and to notify the Department of Health, you must complete the following steps:

Step 1: Review The New York State Department of Health Interim Guidance for End of Academic Year Celebrations During the COVID-19 State of Emergency here.

Step 2: Submit your form, found here, to provide the required information and verify that you have read and understand the obligations to operate under New York State’s guidelines.

Step 3: Implement the required actions for your event including, but not limited to:

  • Collecting Contact Information for Tracing.
  • Conducting Screening Testing for Attendees.
  • Conduct Health Screenings of all Persons attending the event.

Step 4: Enforce Distancing, Face Covering and Activity requirements during the event.

Step 5: Maintain a record of the Contact Information and Tracing information for a minimum of 28 days and provide the data to State and Local Health authorities upon request.
