Fingerprinting Procedure: Mail your completed application and proper payment to the Office of Consumer Affairs, 240 Old Country Road, Mineola, NY 11501. Once your application has been received and reviewed, a staff member will contact you to set up an appointment to be fingerprinted.
Complaints Must Be In Writing: All complaints must be received in writing, signed and dated. CLICK HERE to download and print a complaint form. Once the form and backup documentation have been received, your case will be assigned to an investigator for review and appropriate action.
