ABANDONMENT OF NON-LEAKING UNDERGROUND TANKS:

Homeowner Statement: 
Use the Bureau of Environmental Protection Affirmation of Non-Leaking Tank form (.pdf) seven days in advance of the abandonment or removal to notify the Nassau County Department of Health in writing.

Contractor Statement: 
Have your contractor submit the Small Facility/Homeowner Tank Abandonment Notification Form seven days in advance of the abandonment or removal to the Nassau County Department of Health.

Aboveground or Basement Tank Removals:

Please Note: Original Affirmation Form must be notarized. Completed forms with appropriate fee must be mailed or delivered in person to the Bureau of Environmental Protection, Nassau County Department of Health, 200 County Seat Drive, Mineola, New York, 11501.

About the Medical Reserve Corps MRC Application

The idea for a national volunteer corps emerged out of the tragic events of the 9/11 terrorist attacks. Thousands of medical and public health professionals, eager to volunteer, overwhelmed local responders. Many highly skilled people were turned away. As a direct result of this need, the MRC was launched in 2002.
