Reporting Requirements
Over five dozen communicable diseases must be reported to the Health Department by physicians, nurses and laboratories. If you have a question about these diseases, please call (516) 227-9496 weekdays from 9:00 a.m. to 4:45 p.m. For weekend or after-hours physician consultation, emergency disease reports and food poisoning cases, call (516) 742-6154.

ECLRS
The Electronic Clinical Laboratory Reporting System (ECLRS) provides laboratories that serve New York State with a single electronic system for secure and rapid transmission of reportable disease information to the New York State Department of Health (NYSDOH) and county health departments.

Animal Bites
Medical providers treating a person who has suffered an animal bite are required to report the incident to the Department. To report a bite incident, please use this electronic form or call 516-227-9663 during normal business hours or 516-742-6154 after business hours and on weekends.
